Passport Book:

For persons 15 & younger (Valid 5 years):

Total Cost = $85.00

$25.00 Payable to “Polk County”

$60.00 Payable to “U.S. Department of State” 

For persons 16 & over (Valid 10 years):

Total Cost = $100.00

$25.00 Payable to “Polk County”

$75.00 Payable to “U.S. Department of State”

Expedite Service for faster processing:  Additional fee for expedited service is $60.  The customer must provide a postage paid overnight or special envelope if needed.  This fee is paid to the “U.S. Department of State” along with regular passport fees. 

Passport Card: For adults who already have a passport book, they may apply for the card as a passport renewal by mail and pay only $20.  First-time applicants will pay $45 for adult cards and $35 for children.  Click here for more info.

Passport Photos: $10.00 for two(2) identical 2x2 passport photos.

Notarized Passport Copies: $5.00 each

Acceptable forms of payment:

• Personal check

• Bank draft

• Cashier’s draft

• Certified check

• Traveler’s Check

• Commercial money order (American Express Co, etc)
