ADDITIONAL FEES

Expedite Service for faster processing: Additional fee for expedited service is $60. The customer must provide a postage paid overnight or special envelope if needed. This fee is paid to the “U.S. Department of State” along with regular passport fees.

Passport Photos: $10.00 for two(2) identical 2x2 passport photos.

Notarized Passport Copies: $5.00 each

PAYMENT INSTRUCTIONS

The Application Fee is a non-refundable processing fee and is retained by the Department of State whether or not the passport is issued. The Application Fee and the Agent fee are paid separately at the time of application. The following payment methods are accepted:

Application Fee: Personal checks, money orders and bank drafts payable to "Department of State"

Agent Fee: Cash or Personal checks made payable to "Polk County Recorder"
