ADDITIONAL FEES

Expedite Service for faster processing:  Additional fee for expedited service is $60.  Customer may also pay an additional $12.85 overnight delivery fee to the US Department of State for return delivery of their passport. 

Passport Photos: $10.00 for two(2) identical 2x2 passport photos.

 

Notarized Passport Copies: $5.00 each

 

 

PAYMENT INSTRUCTIONS

 

The Application Fee is a non-refundable processing fee and is retained by the Department of State whether or not the passport is issued.  The Application Fee and the Agent fee are paid separately at the time of application.  The following payment methods are accepted: 

 

Application Fee: Personal checks, money orders and bank drafts payable to "Department of State" - NO CREDIT CARD PAYMENTS ARE ACCEPTED FOR APPLICATION FEE

 

Agent and Photo Fee: Cash or Personal checks made payable to "Polk County Recorder".  Credit (Visa, MasterCard, Discover) payment also available for Agent and Photo fee only.
